    Team Nomination Form

    CATEGORY DESCRIPTION: TEAM

    • Must have competed with distinction at the highest levels of athletic competition at the time of the teams' participation while representing Mustangs Athletics.
    • Highest level should be interpreted to the standards of achieving conference championships, or national championship finalist, or equivalent.
    • Consideration for selection will be given to a nomination in the team category following a minimum 10 year waiting period following the teams' accomplishments.
    • Please be as thorough as you can.
    • If a completed nomination is received after the current year's deadline, the completed nomination will be held in consideration for the next year's induction.

    • Name of the team members - their availble address, phone number and email address.
    • Names of coaches, managers, trainers, athletic director, major sponsors - their available address, phone number and email address.
    • Other important or noteworthy individuals involved with the team.

    Nomination Notes

    • The nomination form is important. The selection committee relies heavily on the content when selecting teams for the Western Mustangs Sports Hall of Fame.
    • The nomination will be added to a file of nominees that is reviewed yearly by the selection committee.
    • The selected Teams will be recognized and honoured at the Western Mustangs Sports Hall of Fame Dinner on the Friday night of Homecoming
    • Pictures of the inducted Teams are displayed on the Western Mustangs Sports Hall of Fame wall in Alumni Hall.
    • You will receive confirmation that your submission has been received via a separate email.
